New hypoglossal nerve stimulator codes expected in April 2026 I/OCE update

CMS posted a note in its MLN Connects newsletter to clarify coding questions related to hypoglossal nerve stimulators used to treat obstructive sleep apnea. Organizations have raised concerns to the agency that the existing Current Procedural Terminology (CPT®) codes don’t accurately describe some of the newer devices.

To address this issue, CMS plans to add six new Healthcare Common Procedure Coding System (HCPCS) codes to the April 2026 Integrated Outpatient Code Editor (I/OCE) update. The new HCPCS codes, which are effective January 1, 2026, are as follows:

  • C8007: Open implantation of hypoglossal nerve neurostimulator array and pulse generator, not requiring insertion of a separate distal respiratory sensor electrode or electrode array
  • C8008: Revision or replacement of hypoglossal nerve neurostimulator array including connection to existing pulse generator
  • C8009: Removal of hypoglossal nerve neurostimulator array and pulse generator
  • C8011: Open implantation of hypoglossal nerve(s) neurostimulator electrode array(s) and receiver, including external power source and all system components
  • C8012: Revision or replacement of hypoglossal nerve(s) neurostimulator electrode array(s) and receiver
  • C8013: Removal of hypoglossal nerve(s) neurostimulator electrode array(s) and receiver

Revenue integrity professionals should take note of the new codes and monitor for the April 2026 I/OCE quarterly files, which should be released in mid-to-late March.
