Charge entry standards: ED charges least likely to be entered by clinical staff

Emergency department (ED) charges are the least likely to be entered by clinical staff or triggered by clinical documentation, according to the preliminary results of the 2025 State of the Revenue Integrity Industry Survey. The survey, which collects information on revenue integrity processes, functions, standards, goals, and more. The survey is open until March 31.

The survey covers key revenue integrity domains including:

  • Chargemaster maintenance
  • Charge reconciliation and capture
  • Denials management
  • Claim edits and billing

Respondents were also asked what types of charges are not entered by clinical staff or triggered by clinical staff members’ documentation. According to the preliminary survey results, the following are the top five types of charges least likely to be entered by clinical staff or triggered by their documentation:

  • ED/trauma department (37%)
  • Drug administration (33%)
  • Observation hours (30%)
  • Room and board (25%)
  • Cardiac cath lab (24%)

The survey closes March 31, so the final results may vary from these preliminary results. The final results will be published in the 2025 State of the Revenue Integrity Industry report and webinar, both of which will be released during 2025 Revenue Integrity Week (June 2-6); the survey will also be used to develop educational resources and identify priorities for NAHRI over the coming year.
